Dr. Gary Holland - Medical Director
In 1983, Dr. Gary Holland graduated Magna Cum Laude from Hampden-Sydney College in Virginia with a bachelor’s degree in Biology. He then attended theMedical College of Virginia, graduating with his MD in 1987. Dr. Holland completed his residency in 1990 in Boise Idaho and was Board Certified in Family Medicine.
In 1998, Dr. Holland moved to Pastaza, Ecuador where he became fluent in Spanish and volunteered his skills at a Christian mission hospital. Upon returning to the United States in 2002, Dr. Holland began specializing in hospice and palliative care. As the full-time Medical Director of Home Medical Services, he continues to render his incredible talent and bedside manner to its deserving patients, in both English and Spanish. Dr. Holland’s dedication and attention to each patient’s physical, emotional, and spiritual care provide hope in times of pain and sickness.
Melanie Muma - Nurse Practitioner
Melanie is a certified Family Nurse Practitioner (FNP-C) with a primary focus in geriatric care. She spent 12 years working as a nurse, primarily in Emergency Medicine. She has also worked in long term care, rehab, and peri-op. She obtained a Master’s degree in Nursing Education and taught college nursing courses for five years. She then obtained a Master’s certificate as a FNP and left the classroom to help patients. She is passionate about treating patients, with a focus on forging caring relationships and educating both they and their family. She proudly joined the U.S. Navy Reserves in 2011, and has worked with military members in many military facilities, including serving in Afghanistan. Melanie enjoys working in the geriatric community seeing patients where they reside. She feels that caring for a patient in their environment is the best way to allow a holistic approach.
Jennifer Fox - Nurse Practitioner
Jennifer is a AANP board certified family nurse practitioner practicing in geriatric medicine in the Davis/Weber area. Jen graduated from Westminster College in 2012 with a Master of Science in Nursing. She has experience in urgent care, women’s and men’s health, pediatrics, geriatrics, chronic illness management, and minor surgical procedures.
Jen was a registered nurse at the VA hospital, in the medical ICU, prior to becoming a nurse practitioner. She has a passion for caring for veterans and it is where her love of caring for geriatric patients started. Jen had the dream of practicing medicine since she was a child. She was driven to pursue advanced medical training from a young age. She has always enjoyed caring for others and wanted to be able to do this in a larger capacity, which is what led her back to a graduate school. Jen takes a holistic approach to patient care and believes that it is important to treat the entire person-physical, emotional, and spiritual needs-and not only the illness.
Jen enjoys learning. She believes in empowering her patients to take charge of their own health and believes that this can be accomplished by supplying patients and families with the most up-to-date information so that they are able to make informed decisions regarding their health.
In her free time, Jen enjoys spending time with her two children, traveling, spending time in the sun and water, and experiencing new activities.
Amy Fowers - Nurse Practitioner
Amy Fowers is board certified with the AANP as a Family Nurse Practitioner (FNP), working primarily with geriatric patients. She graduated from Chamberlain University in 2021 with her Master of Science in Nursing. Amy has been working as an FNP with Home Medical Services since 2021, giving amazing care to the patients in the Weber and Davis county area. She is also working with Holland Holistic, offering a range of natural services; and as the Director of Patient Care Services at A Plus Healthcare. Amy has a genuine passion for caring for the elderly community. She thrives in a holistic approach with her patients, treating the patient's emotional, physical and spiritual needs, promoting overall health and wellness.
Amy started out as a Certified Nursing Assistant in 2007 and later went on to nursing school, graduating in 2011 with her Associates of Science in Nursing. In 2019 she earned her Bachelor of Science in Nursing. Amy worked for many years as a registered nurse managing several assisted living facilities and hospice and home health agencies along the Wasatch Front. She enjoys learning and keeping up with the most up-to-date, evidence-based care.
In Amy’s spare time she enjoys scenic rides in her side-by-side, biking, and traveling.
